All testing accommodations will be provided through Disability Services unless the instructor chooses to
provide the necessary accommodations. Exams may begin as early as 8:00 a.m. and must be completed by
4:30 p.m. Monday - Friday.
The form below must be completed at least three days prior to the exam. The form will generate an
automated request for accommodation email which will be delivered to the professor indicated. It will
then be the professor's responsibility to make arrangements with Disability Services. If there are any
